What this means for your family

The facility is highly regarded for its clean environment and an exceptional hospitality team that keeps residents engaged. However, because multiple reviewers have reported serious medication errors and communication gaps, we strongly advise families to be highly involved in clinical oversight and to verify that medication schedules are being followed correctly.

The Subacute at Autumn Lake Healthcare receives overwhelmingly positive feedback for its clean, well-maintained environment and a highly praised hospitality team, particularly directed by Nikki Spencer. While many families report excellent rehabilitation outcomes and attentive nursing care, a small but vocal minority of reviewers cite critical lapses in medication management, poor communication, and inedible food quality. Families should be aware that while the facility is often described as a 'clubhouse' for recovery, there are reports of inconsistent care standards during off-hours.

Questions for Your Tour

  • 1I noticed your team is very active in responding to online feedback; how do you use that communication loop to address concerns regarding clinical care or daily operations?
  • 2Given that medication management is a critical part of resident health, what specific protocols or double-check systems do you have in place to ensure accuracy and consistency?
  • 3With your focus on physical and occupational therapy, how do you ensure that the nursing staff coordinates effectively with the therapy team to keep residents on track with their recovery goals?
  • 4I understand that dining is a major part of the resident experience; what steps are you taking to improve the variety and quality of the meal options provided?
  • 5Could you walk me through the process for how nursing staff is alerted to call bells and what your standard response time expectations are for non-emergency needs?
  • 6Since your facility has a high capacity of 124 residents, how do you ensure that families receive consistent, proactive updates regarding their loved one's health status and care plan changes?

Key Review Excerpts

My dad is in this facility and I love the communication that they offer with letting me know what is going on. Nikki, the hospitality manager, as well as Karen and Victoria have been nothing but amazing throughout this whole process and very professional.

Memory care family member · 2026★★★★★

Still a patient. While the facility is nice the staff is incompetent. My first day here they hit me a giant dose of insulin when I didn't need it. (15units on a 141 blood sugar) sent me crashing to the low 50s while shaking and trying not to pass out in my room hit the call bell repeatedly for almost 20min.

Rehab patient · 2025★★☆☆☆

My mom is very happy here. It’s really clean and the food is actually good unlike most places. Christy the nurse manager is really good to my mom and I would not be able to sleep at night if it wasn’t for Linda, my mom’s aide on second shift.
