What this means for your family
This facility's therapy team is highly regarded for post-surgery recovery, but the nursing care consistency is a major concern. If you choose this facility, ensure you have a plan for frequent, unannounced visits to monitor your loved one's hygiene and safety, as multiple families have reported neglect when they were not present.

Key Review Excerpts
“The low rating is for the nursing care. Every time I come to visit, I flag safety risks. Today our 97 year old loved one was alone in her room sleeping while her bed was fully raised to its highest setting off the floor and while all the brakes on her bed were off.”
“The facility is nice but dirty. Over the weekend there were dirt and crumbs in the common area. 3 days and did not see housekeeping. I was there to visit frequently. Staff acted like it hurt to move. I know they are short staffed but when you could find one they were in a corner on there phones.”
“My mother was released from hospital to this facility. Her health declined quickly. The doctor never communicated or worked a plan. So she given food she wasn't supposed to eat etc.”

Two complaint-triggered deficiencies indicate families have reported concerns about infection control and discharge procedures at this facility. The most recurring problems involve resident rights violations (particularly discharge notification issues), infection control, and building safety systems. While most deficiencies have been corrected, the pattern shows ongoing challenges with fundamental care standards across multiple surveys, suggesting families should carefully evaluate whether this facility meets their quality expectations.
