The Preserve has a strong overall Medicare rating. RN hours meet the EveryPlace reference benchmark, which is one useful staffing signal to discuss during a visit. Public reviewers frequently mention: highly effective and professional pt/ot therapy team and clean, modern, and well-maintained physical facility. Review the component ratings and current source records before deciding.
The Preserve is widely praised for its modern, clean facility and a highly effective rehabilitation therapy team that many residents credit for their successful recovery. While many families report excellent, compassionate care, there are serious, recurring reports from some families regarding neglect, poor communication, and understaffing, particularly concerning basic hygiene and response times for call lights.

Key Review Excerpts
“The Preserve is easily the best rehabilitation facility I have been in and this is NOT my first rehab. The physical facility is quite nice with large rooms equipped with big flat screen TVs. The nurses and CNAs are friendly and helpful.”
“My father was in the memory care unit for almost 2 years. Jen and the staff truly loved and took care of him. At first, he resisted going into any facility. But quickly he felt cared for and a connection to the other residents and the people taking care of him.”
“The nurses were great but the aids were terrible, wouldn’t help her go to the bathroom, go to bed, eat, she ended up with a severe UTI which has never cleared up. This looks like a high end rehab place but in reality they really lack people and care skills it is shocking.”
This facility meets both EveryPlace staffing reference benchmarks. Higher staffing is generally associated with stronger day-to-day care.
Reference benchmarks (0.75 RN and 4.1 total nursing hours per resident/day) are comparison targets, not current federal minimum requirements.
