Key Review Excerpts

The facility is very clean and the admissions department is great and answered all my questions. I definitely recommend sending your loved one here.

Family member · 2026★★★★★

The therapy team goes above and beyond in improving the patient's quality of life and returning them to their prior levels of functioning and mobility. Lots of fun activities are provided daily.

Visitor · 2025★★★★★

One star for the good work of the week day staff including PT and aides. But this is a dangerous, dehumanizing, run down place for your loved one. Especially if you're not there looking in on them. Weekends are warehousing patients through over medication.

This facility has concerning patterns with families filing complaints leading to serious deficiency findings, including recent issues with resident abuse/neglect protection and accident prevention. The most recurring problems involve daily care assistance, accident hazard prevention, medication management, and food safety/nutrition services. While the facility has corrected all cited deficiencies, the repeated issues across multiple surveys and ongoing family complaints through 2025 suggest persistent care quality challenges that warrant careful consideration during any visit.
