No evidence of completed staff training on reporting, personal care, dementia, and fire prevention was found in the files for Employees 3, 4, 5, and 6.
Failure to maintain a record of the meal census, including the kind and amount of food used for the preceding 3-month period.
Medication administration records (MARs) for several residents (A, B, and E) lacked a specified reason or diagnosis for the use of as-needed (PRN) medications. Additionally, Resident E's MAR showed blank medication entries, and Resident B had three separate prescriptions for Tylenol in one month.
The January 2025 schedule lacked the designation of one person to be the supervisor of resident care for each shift.
