For Staff A, a registry check was only performed on the day of the survey because no prior check could be located.

The facility failed to ensure that two of the sampled staff members (Staff B and Staff C) were tested for Tuberculosis disease upon hire according to required control measures. Specifically, there was no documentation of a second-step TB skin test within 12 months for the identified staff.
