The facility failed to ensure that a resident's care plan included a dated signature from the primary care provider (PCP) within 15 days of completion. Specifically, for one sampled resident, the care plan was signed by the Registered Nurse but lacked the required physician signature. Staff admitted to an oversight in following up with the PCP after faxing the documentation.
