The facility initially received a 'Disapproved' status on 3/18/2025 following a complaint inspection (Ref #170723) regarding generator transfer issues. A subsequent inspection on 06/23/2025 confirmed that all previously noted violations had been corrected, resulting in an 'Approved' status.
During annual testing on 3/10/2025, it was discovered that the transfer switch would not manually transfer from city power to generator power without manual switching.
Facility lacked a formal policy and procedure for manual transfer of power and associated documentation.
