This report details an investigation into three allegations: lack of hot water, uncomfortable temperatures, and poor food quality. The investigator found that all allegations were unsubstantiated after reviewing facility logs, conducting staff and resident interviews, and performing facility tours. The inspection was an unannounced Case Management - Incident visit following a self-submitted incident report. Two Type A deficiencies were cited related to resident rights and personnel reporting of abuse, both posing immediate health risks. The facility was required to develop a Plan of Correction for both violations.

This report details an investigation into a complaint received on June 20, 2025, regarding alleged staff neglect and policy violations. The Licensing Program Analyst found that there was not a preponderance of evidence to prove the alleged violations occurred, resulting in the allegations being unsubstantiated.
