I heard an aid tell residents she was not giving them a shower because she didn't want to. The perineal care was definitely not the best the hall on the top right smelt like poop and tuna fish.

4 complaint-triggered

This facility has recurring issues across multiple areas, with families filing four complaint-triggered reports concerning resident abuse, wrongful use of belongings, staffing credentials, and care planning participation. The most persistent problems involve food safety and handling (cited four times across three years), care planning deficiencies, and medication management issues. While all violations show correction dates, the pattern of repeated food safety violations and recent complaints about abuse protection raise concerns about sustained quality improvements.
