What this means for your family

Stonebridge shows strong performance in rehab and social engagement, which is great for short-term stays. However, given the serious reports of hygiene and care inconsistencies, I strongly recommend conducting unannounced visits during weekends or evenings to observe the actual level of staff attentiveness.

Key Review Excerpts

The food is high-quality, nutritious, and appetizing for the residents. Stonebridge has a dedicated Activities Team which does a stellar job offering activities to engage, stimulate, entertain.

Long-term resident's family · 2026★★★★★

I would not recommend sending your loved ones to this place. My grandpa was sent there since he was in the late stages of Alzheimer’s. He was only there for 3 weeks and his clothes were lost and gave to other patients.

Memory care family member · 2021☆☆☆☆

My mother in law was at this facility for a time a few years ago and i would not recommend it to anyone who wants Compassionate care and kindness. She would call us from her cell phone in the middle of the night and my husband and I had to go up there to change her diapers.

3deficiencies
Well below state avg (7.6)

This facility shows a mixed record with 15 deficiencies across three surveys, primarily in fire safety, medication management, and resident assessment areas. Most issues appear corrected by the facility, though fire safety violations have recurred across multiple inspections, including recent October 2024 findings for blocked exits and faulty smoke systems. While all deficiencies have correction plans in place, families should inquire about ongoing fire safety improvements during visits.
