Direct care staff were performing assigned tasks without having required training (reporting, first aid, CPR, etc.) completed prior to performing tasks.

The licensee was not obtaining physician health statements on direct care staff within 30 days of hire.

Direct care staff (Jasmine Foster) subjected a resident to mental/emotional cruelty, verbal abuse, derogatory remarks, and threats by calling the resident names and threatening removal from the home.

Insufficient food supply; residents complained of hunger and being forced to eat substitutes due to strict grocery budgeting; food supply was not enough to sustain residents beyond a few days.
