The facility failed to maintain hot water temperatures at a minimum of 100 degrees F and a maximum of 116 degrees F for 4 out of 5 sampled fixtures in resident bathrooms. Observations and logs revealed temperatures as high as 131.2 degrees F, and several fixtures lacked required warning signage for hot water.

The facility failed to ensure residents were offered activities designed to promote active involvement on a daily basis. Observations and interviews revealed that while an activities calendar was posted, no activities were initiated by staff, and residents had to initiate their own activities. Residents also noted a lack of outings and that activities had declined since the departure of the Activity Director.

The facility failed to meet requirements regarding the training and competency of medication aides. Specifically, the facility is prohibited from allowing staff to perform unsupervised medication aide duties without proper training or recent experience in an adult care home.
