What this means for your family

While the physical therapy department is frequently praised, the recurring reports of severe neglect, long wait times for basic needs, and medication errors are deeply concerning. If you choose this facility, we strongly recommend that a family member or advocate be present frequently to ensure basic care needs are met and to monitor communication with the nursing staff.

Key Review Excerpts

The facility was clean and the staff very nice. Good security at check-in. I would recommend Peak.

Rehab patient's family · 2022★★☆☆☆

My grandmother who is a fall risk would leave her light on at least an hour before receiving help most of the time. She would never get her medicine on time and the nurses never came to check on her at any point.

Long-term resident's family · 2024★★☆☆☆

The OT and PT care was adequate. However, the rest of the care was well below par. He would lie in bed for hours before anyone came to change his Depends, even after pushing the call button.

This facility shows concerning patterns with families filing complaints that led to multiple deficiencies, including repeated safety hazards and accident prevention issues found twice within eight months. The main problem areas involve resident safety and accident prevention, nursing staffing requirements, and facility maintenance including heating systems and medical gas storage. While the provider has correction dates for most issues, the recurring safety problems and multiple family complaints suggest ongoing care quality concerns that potential visitors should carefully consider.
