During the survey, it was observed that the medication aide (MA) was performing food service duties by plating food, and the personal care aide (PCA) was serving food to residents. This practice may violate regulations regarding the separation of aide duties from food service responsibilities.

The facility failed to maintain staffing levels and duties in accordance with regulatory requirements for residents. Specifically, the regulations dictate strict limitations on aides performing housekeeping and food service duties, as well as specific staffing ratios for aides, supervisors, and administrators based on resident census.

The facility failed to serve water to residents at each meal as required by regulation. Observations showed no water was placed on dining tables during lunch on 09/04/24 and breakfast on 09/05/24. Interviews with dietary staff and management confirmed they were either unaware of the requirement or only provided water upon request after meals had begun.

The facility failed to ensure therapeutic diets were served as ordered for two residents requiring low-fat/low-cholesterol diets. Observations revealed the refrigerator lacked egg substitutes and the freezer lacked sherbet, despite these being listed on heart-healthy menus. Additionally, menu items for the heart-healthy diet did not consistently align with the required nutritional substitutions.
