The facility failed to ensure that one of six staff members authorized to administer medications had completed the required clinical skills validation. This lack of documented competency occurred prior to the staff member administering medications to residents.

The facility failed to ensure that two out of six sampled medication aides completed the required state-approved medication aide training and testing. Specifically, Staff B lacked documentation for the 5-hour, 10-hour, or 15-hour training and the required medication aide test, despite administering medications to multiple residents.

The facility failed to ensure that medication aides completed required state-approved training and passed the required written examination. Specifically, two of six sampled staff members were found to be administering medications without documented completion of the 5, 10, or 15-hour training and the medication aide test.

The facility failed to ensure that two medication aides (Staff C and Staff D) had a current medication clinical skills checklist completed for this facility prior to administering medications. Although the staff had completed checklists at previous employers, there was no documentation of a valid skills validation on file at the current facility.

The facility failed to provide increased supervision for one resident who had experienced recurrent falls. The facility did not adequately implement or adjust interventions based on the resident's assessed needs and fall history.

The facility failed to ensure that 2 of 6 medication aides had a current medication clinical skills checklist completed for this facility prior to administering medications. Personnel records for Staff C and Staff D showed no documentation of a clinical skills checklist being completed at this specific location, despite both staff members administering medications in July 2023.
