The facility failed to maintain hot water temperatures between 100 and 116 degrees Fahrenheit for 8 out of 15 fixtures. Specifically, temperatures in the Men's Hall common bathroom and resident rooms were recorded as high as 128 degrees F.

The facility failed to maintain clean and well-repaired floors in shared bathrooms between resident rooms 101/103 and 105/107. Observations revealed broken and loose floor tiles near commodes, grimy discoloration, and a strong odor of old urine. Maintenance and staff had not identified or addressed these repair needs.

The facility failed to ensure that 3 out of 7 sampled staff members had completed the required two-step Tuberculin skin test. Specifically, one staff member had no record of any TB testing, while two other staff members had completed the first step but lacked documentation for the second step.

The facility failed to maintain proper oversight of TB testing protocols for new hires. Interviews revealed that the Business Office Manager and Administrator had inconsistent understandings of the required timeframe for completing the two-step testing process for employees.

The facility failed to ensure that 3 out of 7 sampled staff members had completed the required two-step Tuberculosis (TB) skin test. Specifically, Staff A, Staff B, and Staff F had documented step 1 tests but lacked the required step 2 documentation in their personnel records.

The facility failed to provide increased supervision in accordance with the assessed needs, care plans, and current symptoms for two residents with multiple falls, including falls requiring Emergency Medical Services. Specifically, for one resident, the care plan was not updated following changes in medical status, and there was a lack of appropriate monitoring despite high-risk diagnoses and recent falls.
