What this means for your family

While the facility is praised for its clean environment and effective therapy team, the recurring reports of weekend understaffing and neglect are serious red flags. If you choose this facility, ensure you have a family member present frequently, especially on weekends, to monitor hygiene and hydration.

Key Review Excerpts

PT, OT , Social Worker, and activity director were great. But, under staffed and some medical personnel and CNAs acted like they couldn't care less about their patients.

Rehab patient family member · 2023★★★☆☆

My grandfather went here to do rehabilitation, he was supposed to have therapy 6 days a week but on the weekends was left in a diaper all weekend and told just to go in the diaper and they’d clean him up when they could.

Grandchild of rehab patient · 2024☆☆☆☆

My grandfather was here for 25 days after major surgery and had to be re-hospitalized for infection, dehydration, and poor nutrition. In my opinion, this facility is not currently equipped to help someone heal after major internal surgery.

7 complaint-triggered

This facility shows concerning patterns with families filing complaints that led to 7 deficiencies, most recently in February 2025. The main problem areas are resident assessment and care planning (which appear across all three surveys), building safety issues, and medication management. While the facility reports correcting all violations, the persistence of assessment problems and recent complaints about respiratory care and privacy protection suggest ongoing care quality concerns that warrant careful evaluation during any visit.
