What this means for your family

While the therapy department is frequently praised for its effectiveness, the facility has a recurring pattern of serious neglect regarding basic patient care and hygiene. Families considering this facility should conduct unannounced visits during weekends or evenings to observe staffing levels and response times, as these are the times most frequently cited in negative reports.

Google Reviews

Google Reviews

91 reviews on Google
Signature Healthcare of Norfolk receives highly polarized reviews, with many families reporting severe neglect, poor hygiene, and unprofessional staff behavior, while others praise the therapy department and specific nursing staff. The facility faces recurring complaints regarding slow response times for basic needs, such as toileting and medication, as well as concerns about the physical condition of the building. Potential families should be aware of the significant disparity between the positive experiences of some rehab patients and the distressing reports from long-term care families.

Questions for Your Tour

  • 1I noticed your team is very active in responding to online feedback; how does that commitment to communication translate into how you keep families updated on their loved one's daily health status?
  • 2Given the feedback regarding hygiene and toileting assistance, could you walk me through the specific protocols and oversight in place to ensure residents receive timely help with these daily needs?
  • 3I see your therapy team is highly regarded for being encouraging; how do you ensure that same level of attentiveness and positive interaction carries over to the nursing and CNA staff on the floor?
  • 4With the facility infrastructure being a bit older, what is your current plan for maintaining cleanliness and managing odors in the common areas and resident rooms?
  • 5I understand that meal service can be a challenge; what steps are you taking to improve the quality of the dining experience and ensure that residents’ dietary preferences are consistently met?
  • 6Can you describe your process for handling medical emergencies or sudden changes in a resident's condition, especially during overnight hours when staffing levels may be different?

Key Review Excerpts

I had a amazing experience in therapy and they didn't push me to the point of more pain. I would recommend people to be placed here. They offer daily activities for the patients and does not force you to participate

Rehab patient · 2025★★★★★

The entire building stinks of urine, and staff frequently cuss at patients and is very disrespectful to them. Patients are not assisted with bathroom needs in a timely way and are often left in soiled adult briefs for hours

Family member · 2025☆☆☆☆

My mother went to Signature almost three weeks ago for simple physical therapy. Within two weeks, she had to be rushed to the ER with severe dehydration, perilously low blood sugar and COVID.

This facility has a concerning pattern with 65 deficiencies across three surveys, including families filing complaints about medication errors, daily care assistance, and living conditions. The most recurring issues involve resident rights violations, care planning problems, and medication management. While the facility corrects identified problems, the high volume of deficiencies and complaint-triggered violations suggest ongoing quality challenges families should carefully consider.
