The facility failed to maintain a clean and orderly environment free of hazards. Specifically, resident care items and mobility aids were improperly stored in spa areas, and boxes were stacked on the top shelf of the dry food storage room, obstructing the fire sprinklers.

The facility failed to ensure physician notification and follow-up for a resident's routine health care needs. Specifically, the physician was not notified that Resident #1 had refused 11 weekly doses of alendronate sodium between June 26, 2017, and October 2, 2017.

The facility failed to ensure that medications were administered in accordance with physician orders for 2 of 7 sampled residents. Specifically, for Resident #2, the facility failed to update the Medication Administration Record (MAR) with a new order for warfarin 2 mg daily and failed to accurately document doses, leading to staff unawareness of correct dosing instructions.

Specifically, for Resident #2, medication records showed incorrect dosages of Coumadin (warfarin) being administered and a failure to update the Medication Administration Record (MAR) following a change in physician orders.
