The facility failed to maintain a resident's bedroom and shared bathroom in the special care unit in a clean and orderly manner. Observations revealed soiled bedsheets, dirt on the mattress, cigarette butts and ashes on the floor, and significant staining and odors in the bathroom.

Observations revealed soiled bedding with brown stains, thick dust on the mattress, and debris on the floor. Additionally, the bathroom contained cigarette ash, burn marks, and various brown stains on the walls and toilet.

Specifically, the posted diet list was not dated and incorrectly listed Resident #3's diet as mechanical soft for meats only. This resulted in the resident being served foods that did not meet the required texture specifications for their prescribed diet.

The facility failed to ensure that all therapeutic diets, including food consistency, were served as ordered by the resident's physician. For one resident with a mechanical soft diet order, observations showed that only certain items were chopped, failing to meet the required dietary consistency.
