Key Review Excerpts

My father was brought here after he had a triple bypass. He has dementia. His catheter broke and they left it for HOURS. He had a feeding tube and when he was admitted, they didn't feed him for TWO DAYS.

Long-term resident's family · 2022☆☆☆☆

The rehab is okay but once you get to the care side it’s abysmal. The DON will lie to your face. Keep your loved ones out of here; however, if you can’t GET A CAMERA for the room it will at least be a deterrent.

Family member · 2025☆☆☆☆

My dad was here for rehab and had such wonderful care. Staff was very caring and worked with him to improve so he could come home safely.

This facility shows concerning patterns with multiple family complaints triggering inspections, including recurring issues with safety supervision that appeared three times and resulted in actual harm to residents. The most frequent problems involve care planning, nursing staff competency, and resident safety, with additional issues in medication management and environmental safety. While all deficiencies show correction dates, the repeated safety violations and high number of complaint-driven investigations suggest ongoing care quality concerns that families should carefully investigate before placement.
