The facility failed to ensure that all exit doors were easily operable by a single hand motion from the inside without keys. Specifically, the metal push bars on the double exit doors at the end of the hallway were chained and padlocked together, preventing the doors from opening. This created a major fire safety hazard for residents in rooms 5 through 15.

Specifically, temperatures were found to be as low as 70 degrees Fahrenheit in resident showers and various bathroom sinks. This deficiency impacted multiple resident bathrooms and the kitchen area.

The facility failed to notify the local department of social services regarding accidents involving two of five sampled residents. Specifically, Resident #1 suffered a fall resulting in a head laceration requiring sutures and EMS transport, and Resident #2 suffered a fall resulting in a tibia/fibula fracture and hospital admission, but neither incident included documentation of notification to the county department of social services.
