The facility failed to ensure that all exit doors were easily operable by a single hand motion from the inside without keys. Specifically, the metal push bars on the double exit doors at the end of the hallway were chained and padlocked together, preventing the doors from opening. This created a major fire safety hazard for residents in rooms 5 through 15.

Specific issues included thick buildup of dirt and dust on ceiling fans, air vents, and floor corners, as well as uncleaned windowsills containing cigarette ash and butts. Additionally, walls and baseboards in several rooms and common areas exhibited scuff marks and brownish smears.
