A relicensure survey was completed on 12/18/24. A deficiency was cited. Based on observation and interviews, the residence failed to provide toilet paper and paper towels in each common bathroom, affecting six current residents.Finding include:On 12/18/24 at approximately 7:30 a.m., an environmental tour of the residence revealed the common area bathroom did not have toilet paper available. On 12/18/24 at 10:32 a.m., Staff #1 stated she was unaware there was no toilet paper in the common area bathroom and acknowledged she should have verified there was toilet paper when she began her shift at 6:00 a.m. On 12/18/24 at 10:50 a.m., the administrator designee stated that residence staff were responsible for ensuring the common area bathroom was stocked with toilet paper, and expected that to have occurred. THIS PORTION OF THE REPORT IS FOR INFORMATIONAL PURPOSES ONLY.No response is necessary.The residence was advised it must review and maintain the following processes in accordance with existing program regulations found at 6 CCR 1011-1, Chapter 7.7.13 Each personnel file shall include, but not be limited to, written documentation regarding the following items: (G) Documentation of initial dementia training and continuing education for direct-care staff members.7.14 If the employee or volunteer is a qualified medication administration person, the following shall also be retained in the employee' s or volunteer' s personnel file: (A) Documentation that the individual' s name appears on the Department' s list of individuals who have successfully completed the medication administration competency evaluation.18.9 The face sheet shall be updated at least annually and contain the following information: (E) Date of admission and readmission, if applicable.
