An initial licensure survey was completed on 10/20/25. A deficiency was cited. Based on observation, record review and interviews, the residence failed to be in compliance with all applicable regulations.This failure created the potential for mismanagement of the care of and services for the residents who would be served by this residence.Findings include:-10.9 Each kit shall include, at a minimum, the following items: (A) Latex free disposable gloves,(B) Scissors, (C) Adhesive bandages, (D) Bandage tape, (E) Sterile gauze pads, (F) Flexible roller gauze,(G) Triangular bandages with safety pins, (H) A note pad with a pen or pencil, (I) A CPR barrier device or mask, and(J) Soap or waterless hand sanitizer.The residence' s undated first aid policy, read in part: "each first aid kit is checked regularly to ensure it is fully stocked and that expiration dates are not exceeded. Each kit includes, at a minimum, the following items: latex free disposable gloves, scissors, adhesive bandages, bandage tape, sterile gauze pads, flexible roller gauze, triangular bandages with safety pins, a note pad with a pen or pencil, a cardiopulmonary resuscitation (CPR) barrier device or mask, soap or waterless hand sanitizer."On 10/20/25 at 10:45 a.m., the administrator provided the residence' s first aid kit. However, the kit was missing a CPR barrier device or mask.On 10/20/25 at 10:46 a.m., the administrator stated she was unaware the first aid kit needed to include a CPR barrier device or mask.-13.1 The assisted living residence shall adopt, and place in a publically visible location, a statement regarding the rights and responsibilities of its residents. The assisted living residence and staff shall observe these rights in the care, treatment, and oversight of the residents. The statement of rights shall include, at a minimum, the following items: (A) The right to privacy and confidentiality. Findings include:The residence' s undated Resident Rights policy, read in part that residents had the right to privacy. Based on observation, record review and interview, the facility (residence) failed to ensure the member (resident) had a right to privacy in their living/sleeping units, affecting all residents who would be served by this residence. Findings include:The residence' s undated Resident Rights policy, read in part that residents had the right to privacy. During the environmental tour on 10/20/25 from 9:00 a.m. to 11:00 a.m., observations and interviews revealed the following:1. Double occupancy rooms provide a retractable privacy curtain that allows complete visual privacy around each bed.The residence failed to ensure double occupancy Room #1 and #7 had privacy curtains.On 10/20/25 at approximately 10:25 a.m., the administrator stated an external contractor was supposed to install privacy curtains in Room #1 and #7 the week prior to the onsite investigation; however, had not. The administrator acknowledged she was aware of the requirement for residents to be provided with privacy.2. WindowsThe windows in Room #1-#7 did not have blinds or curtains for privacy.On 10/20/25 at approximately 10:25 a.m., the administrator stated an external contractor was supposed to install curtains on the windows in Rooms #1-#7 the week prior to the onsite investigation; however, had not. The administrator stated that she would consider the lack of coverage on the windows a privacy concern.3. Single mechanism locksThe common area bathrooms across from Room #1 and #4 were able to be locked from the inside. However, the doors were able to be unlocked from the outside with a fingernail.On 10/20/25 at approximately 9:40 a.m., the administrator stated that she would consider the bathroom locks a privacy concern, since the doors could be unlocked by other residents with a fingernail.
