A relicensure survey was completed on 10/4/23. Deficiencies were cited. Based on observation and interview, the residence failed to ensure the grounds were maintained to protect residents from hazards, affecting 10 current residents.Findings include:On 10/4/23 at 7:58 a.m. to 8:18 a.m., an exterior environmental tour was conducted at the residence, slopes and hazards were identified as follows:The right side of the driveway approaching the house the sidewalk dropped off more than four inches to the grass, which posed a fall risk for residents who used this area to exit toward the front of the residence.The residence' s backyard had a patio t.. Based on observation and interview, the residents failed to ensure resident rooms occupied by smokers were equipped with fire resistant wastebaskets, affecting three of five residents who smoked cigarettes (#2, #4, #5).Findings include:1. ObservationsOn 10/4/23 at approximately 10:23 a.m., Resident #2 ' s room contained one plastic wastebasket with no label indicating it was fire resistant.On 10/4/23 at approximately 10:27 a.m., Resident #4, #5 ' s rooms contained metal mesh wastebaskets which were not fire resistant.2. Based on observation and interview, the facility (residence) failed to ensure the grounds were maintained to protect residents from hazards, affecting 10 current residents.Findings include:On 10/4/23 at 7:58 a.m. to 8:18 a.m., an exterior environmental tour was conducted at the residence, slopes and hazards were identified as follows:The right side of the driveway approaching the house the sidewalk dropped off more than four inches to the grass, which posed a fall risk for residents who used this area to exit toward the front of the residence.The residence' s backyard had a patio that connected with the grass and had a drop off of more than four inches which posed a fall risk for residents who used this area.On 10/4/23, at 9:00 a.m. to 1:00 p.m. several residents were observed utilizing the front and back yard.On 10/4/23 at 1:40 p.m.1:52 p.m., an interior environmental tour was conducted at the residence. The environmental tour revealed an auxiliary aid utilized by one of 10 residents (#3). On 10/4/23 at 1:53 p.m. to 2:13 p.m., the administrator designee acknowledged that the drop off in the front and back yards posed a fall risk for residents who used the areas. Based on observation, interview and record review, the facility (residence) failed to maintain and follow written policies and procedures for the administration of medication in accordance with 6 CCR 1011-1, Chapter VII , Medication Administration Regulations, affecting seven of 10 current participants (residents) (#2-#5, #7-#9).1. Chapter VII regulations governing assisted living residences, part 14.28, requires that the assisted living residence shall ensure that qualified medication administration persons are trained in and apply nationally recognized protocols for basic infection control and prevention when preparing and administering medications.a.
