A relicensure survey with complaint #CO40073 was completed on 2/3/26. Deficiencies were cited. A change of ownership occurred on 10/1/25. Based on observation and interviews, the residence failed to ensure the residents received the cooperation of the residence to achieve the maximum degree of benefit, affecting three of seven sample residents (#1, #5-#7).Findings include:1. Residence Policy The residence' s posted Resident Rights read, in part, "The statement of rights shall include, at a minimum, the following items ... The right to choice and personal involvement regarding care and services, including ... the right to expect the cooperation of the assisted living residence in achieving the max.. Based on record review and interview, the residence electronic medication administration record (eMAR) failed to accurately document each medication administration or monitoring event at the time the event was completed, affecting three of four sample residents (#1-#3) whose medication administration records were reviewed. Findings include:1. Residence PolicyThe electronic medication administration record policy dated, 12/13/24, read in pertinent part: "Any medication that staff members assisted with will be documented on the eMAR/MAR, in accordance with st.. Based on record review and interview, the residence failed to have a roster of current residents readily available, which included emergency contact information and a facility diagram showing room locations, affecting 41 current residents.Findings include:On 2/3/26 at 8:04 a.m., the residence was asked to provide a resident roster with all current residents. However, the document provided did not include room assignments, emergency contact information, and facility diagram. On 2/3/26 at 9:50 a.m., a second request for the emergency preparedness roster .. Based on record review and interview, the residence failed to request, prior to hire, a criminal history record check conducted by the Colorado Bureau of Investigations (CBI) for three staff (#1-#3), affecting 41 current residents. Findings include: On 2/3/26, review of the personnel records for Staff #1-#3 revealed they were hired on 6/7/21, 2/16/25, and 7/5/2004, respectively. However, there was no evidence that a CBI background check had been completed prior to their hire date.During the on-site visit on 2/3/26, from 7:30 a.m. to 4:00 p.m., Staff #1-#3 were .. Based on record review and interviews, the residence failed to ensure each personnel file contained all required elements for three of three sample staff (#1-#3), affecting 41 current residents. Findings include:On 2/3/26 at 8:00 a.m., the complete personnel files as required by Chapter 7, Part 7 for Staff #1-#4 were requested from the administrator.A review of the personnel files for Staff #1-#3 revealed no evidence of:(A) A description of the employee or volunteer duties.(C) Orientation and training, including, but not limited to the following, as applicable. (E) Results.. THIS PORTION OF THE REPORT IS FOR INFORMATIONAL PURPOSES ONLY.No response is necessary.The residence was advised it must review and maintain the following processes in accordance with existing program regulations found at 6 CCR 1011-1, Chapter 7.14.21 The assisted living residence shall be responsible for complying with authorized practitioner orders associated with medication administration except for those medications which a resident self-administers.
