The facility failed to ensure a resident's annual medical examination (FL2) was updated. Specifically, one resident's medical examination had not been completed since October 2023, despite an admission date in November 2022.

The facility failed to ensure the Resident Register initial assessment was completed and signed by the administrator or supervisor-in-charge and the resident or responsible person within 72 hours of admission.

The facility failed to ensure a resident's FL2 medical examination was updated annually. Specifically, Resident #1's medical examination had not been completed since 10/31/23, despite an admission date of 11/04/22.

The facility failed to ensure that all current orders for medications and treatments were reviewed and signed by a physician or prescribing practitioner at least every six months. This deficiency was identified for two of two sampled residents.

The facility failed to ensure the Resident Register was signed by the Administrator within 72 hours of admission for one resident. For Resident #1, while the resident signed the register on 11/01/22, there was no documentation of the Administrator's signature.
