Key Review Excerpts

At one time this may have been a great facility. My mother was here and there was constant problems. Don't get me wrong there was some staff there that tried to do their job but the majority of them did not do their jobs.

Long-term resident's family · 2026☆☆☆☆

The food was terrible; it did not even look like it was editable and would not follow a reasonable diet recommendations (Over processed and carb loaded) Physical therapy would not do their jobs but constantly on their phones instead of working.

2deficiencies
Well below state avg (10.1)

Rest Haven has a mixed inspection record with 9 deficiencies across 3 surveys, all corrected by the facility. Fire safety issues are the primary concern, appearing in both 2022 and 2024 inspections involving sprinkler systems, fire alarms, and cooking facilities. The facility also had problems with resident assessments and care planning in 2024. While all deficiencies were addressed, the recurring fire safety violations suggest ongoing maintenance challenges that families should discuss during visits.
