Key Review Excerpts

My in-laws have been living at RW-C for three years and have received wonderful support from all the years - nursing, culinary and administrative. They have lived independently and in memory care. The memory care nursing is particularly attentive and caring.

Long-term resident's family · 2024★★★★★

Our mother has lived here for exactly 10 years, and us six kids love her living situation. She spent her first six years in independent living (a cottage), then one year in assisted living, and has just moved into a private room in health care. The staff and activities at all levels, are simply amazing, to a person.

Long-term resident's family · 2024★★★★★

After eight years of residence at RW-C, our sons and grandchildren are as happy about us living here as we are. We get to “embrace life on our terms."

Based on record reviews and interviews, it was determined that a signed and dated written agreement with the facility and the resident, or their legal representative, or an acknowledgement of notification specific to the topics outlined in 22VAC40-73-390.A, was not included in each resident?s record. Evidence: 1) A review of the record for resident #3 did not contain a signed and dated written agreement between the resident, or their legal representative and the facility, or an acknowledgment that includes all the required topics outlined under 22VAC40-73-390.A, was in the resident?s record. 2) Resident #3 transferred to the facility from the independent living program operated by the corporation that operates the assisted living facility.
