Hazardous/toxic chemicals and sharp scissors were stored in an easily accessible cabinet/drawer in the common dining room.
Resident B was administered medication in applesauce despite physician orders specifying administration with water.
Resident A did not have a tuberculosis (TB) screening on record performed within 12 months prior to admission.
