Staff were crushing medications and placing them in pudding for a resident without a physician's order in the medication administration record authorizing this method.
Dishwasher sanitization records contained missing or blank entries, making it impossible to verify if utensils were properly sanitized.
Four employees had tuberculosis screenings completed outside the required 10-day window from their date of hire.
