What this means for your family

While the facility has a dedicated administrative team that can assist with complex transitions, the recurring reports of slow call-button response times and hygiene issues are significant red flags. If you choose this facility, ensure you have a strong advocate who can visit frequently to monitor care, and ask specifically about their current staffing ratios on weekends.

PruittHealth-Trent receives highly polarized feedback, with families either praising specific staff members for compassionate care or reporting severe neglect and communication failures. While some families highlight a 'family-like' atmosphere and effective rehab services, others describe a facility struggling with understaffing, poor hygiene, and dangerous lapses in medical oversight. The facility is frequently described as dated, and consistent concerns regarding responsiveness and basic care standards suggest a high level of variability in the quality of care provided.

Key Review Excerpts

While the physical facility is drab and dated, the administrative staff, nurses, aides, physical therapists and occupational therapists have provided professional, excellent, kind, compassionate and timely care to my husband every day.

Rehab patient's spouse · 2026★★★★

My 36 year old daughter had to spend time in this facility. She came very close to losing her life due to the neglect in this place. I'm sure some people have different opinions of this 'skilled rehab facility'. They pride themselves on their wound care, which is a joke.

Long-term resident's family · 2025☆☆☆☆

Annalee, the business officer manager is also a favorite, especially of mine, as she has helped us navigate some confusing medicaid and health care rules and programs. She is quick to respond and find answers we need.

PruittHealth-Trent has a concerning pattern of recurring deficiencies across multiple surveys, with families filing complaints about infection control, medication management, resident rights, and food safety. The facility shows persistent issues with resident assessments, care planning, and building safety that reappear across different inspections. While all violations have correction dates, the repeated nature of problems in core care areas suggests ongoing operational challenges that families should carefully consider.
