While the facility hosts engaging family events and maintains a clean environment, there is a recurring and critical pattern of failing to identify or act upon medical emergencies. If you choose this facility, you must be prepared to be an aggressive advocate for your loved one's medical needs, as multiple families have reported that staff failed to escalate urgent health issues until it was too late.

Key Review Excerpts

I complained for 5 days that something was wrong with my loved one in Soring Valley but the nursing and Dr. did nothing - kept saying he was ok. When Iwent to see him, he was essentially unconscious, delirious, fevered and they had take him to the ER.

Long-term resident's family · 2024☆☆☆☆

I attended the 3-21-24 Family Night activity at Spring Valley. The senior dancers were a hit; residents, families, friends and visitors and (employees) all seemed to enjoy the night’s activity. The facility floors had been stripped and waxed recently and the building overall was in nice, clean shape.

Long-term resident's family · 2024★★★★★

Run as fast as you can from this facility. My father was there for rehabilitation and they did not monitor his health. He died was sent via ambulance to Anderson Hospital on Monday, 4/8/2024 and less than 24 hours later he was dead with Kidney and organ failure.

Clean record — no deficiencies

This facility has faced significant challenges with families filing complaints that led to deficiencies in 2024 regarding resident notification requirements, daily care assistance, and nutrition. The most recurring issues involve care planning and assessment, daily care quality, and building safety systems. While all deficiencies show correction dates, the pattern of complaint-triggered violations suggests families have had serious concerns about basic care standards that required federal intervention.
