What this means for your family

This facility has a highly regarded therapy department that can be excellent for recovery, but the consistency of basic nursing care remains a significant risk. If you choose this facility, be prepared to be highly involved in your loved one's daily care, specifically monitoring medication schedules and hygiene, and ensure you have direct contact information for the nursing supervisor.

Prescott Village Nursing & Rehabilitation (often referred to as Granite Creek in reviews) presents a highly polarized experience for families. While many reviewers praise the facility's recent leadership changes, dedicated therapy team, and specific staff members who provide compassionate care, a significant number of families report severe issues including neglect, delayed medication administration, poor hygiene, and unresponsive communication. Families should be aware that while some patients have successful rehabilitation stays, others have reported critical lapses in basic care and safety protocols.

Key Review Excerpts

I'm a completely different person after my rehab at Granite Creek Rehab. When I first arrived, I was afraid to even touch the ground with a toe. Now, thanks to the fantastic Physical and Occupational Therapy staff, I'm walking and back home.

Rehab patient · 2025★★★★★

My dad was here! during his stay, he was left in bed for hours at a time, supposed to have PT daily after his stroke. He was left to lay in soiled bedding after attempting to Urinate by himself, knowing he wasn't even able to sit or stand on his own.

Long-term resident's family · 2025☆☆☆☆

The new leadership team at this facility are doing great things and the energy is very positive! The staff are friendly and professional and it’s a warm, comfortable atmosphere.
