Key Review Excerpts

The facility is a filthy mess and they do not follow appropriate protocols. They 100% contributed to my grandfather's death by not giving him his medications, sitting up in a chair for hours when he had significant spinal issues, and they did no physcial therapy.

Long-term resident's family · 2024☆☆☆☆

The actual building isn't in the best shape, but the care has been immaculate.

Rehab patient's family · 2025★★★★★

Patients sent there for “ rehab “are over medicated and thus labeled uncooperative or unable to participate. Rather than help patients who may need assistance to relieve themselves on a commode, they are left to defecate in bed and then lie in their own feces.

This facility shows concerning patterns with families filing complaints that led to findings about inappropriate response to abuse allegations and bladder care issues. The most frequent problems involve fire safety violations, nursing staffing shortages, and quality of care deficiencies including daily living assistance and infection control. While all issues have been reportedly corrected, several problems like fire safety, nursing staff adequacy, and abuse prevention policies have recurred across multiple surveys from 2021 to 2025, suggesting ongoing challenges with maintaining standards.
