Peninsula Reflections

This report details an investigation into complaints regarding staff response to resident calls for assistance and communication with emergency personnel. Although allegations were made, the department ultimately found that there was not a preponderance of evidence to prove any violations occurred, resulting in the allegations being unsubstantiated.

The facility underwent an unannounced annual inspection on October 20, 2025. The inspection covered the physical plant, kitchen, medication storage, and resident rooms. No deficiencies were cited during this review, indicating compliance with current licensing standards.

The unannounced case management visit was conducted regarding an alleged incident from 12/6/24. The Licensing Program Analyst reviewed the resident's file and interviewed staff, finding no evidence to substantiate the reported incident. No citations were issued during the visit.

The unannounced case management visit focused on an elopement incident involving Resident 1 (R1). Deficiencies were cited regarding the failure to maintain updated medical assessments for R1 and general supervision protocols following the elopement. One deficiency was dismissed, but two Type B deficiencies remain open requiring corrective action by October 7, 2024.

The facility underwent an Annual 1-year required inspection on September 17, 2024. The physical plant was observed to be clean, and safety equipment, including fire extinguishers and detectors, was noted as operational. No deficiencies were cited during this visit.

This report details two separate complaint investigations conducted on May 14, 2024. The first allegation regarding illegal eviction was found to be unfounded. The second allegation concerning staff supervision resulting in an altercation was deemed unsubstantiated due to conflicting information and lack of evidence.
