What this means for your family

While some families report positive experiences with the rehab and social work teams, the recent, severe reports of patient neglect, hygiene issues, and medical complications are deeply concerning. If you are considering this facility, we strongly recommend conducting an unannounced visit and specifically observing staff responsiveness to call bells and the cleanliness of common areas.

Key Review Excerpts

Refused to answer my father's call bell until I called up there several times. His roommate also used the call bell and was left in his own poop for 3 hours.

Rehab patient's family · 2025☆☆☆☆

Grandma was only here for 4 months and was called and told they sent her to hospital. She was septic and one of the worst bed sores anyone has ever seen.

Long-term resident's family · 2026☆☆☆☆

The staff are the laziest bunch of prejudice folks I have ever seen. The food is awful and the place is nasty. The care is not good at all.

Peak Resources-Shelby shows concerning patterns with multiple uncorrected deficiencies from a February 2024 survey still lacking correction plans, including issues with medication management, resident notification requirements, and infection control. The facility has recurring problems with fire safety barriers and medication labeling across multiple surveys. While some deficiencies have been corrected, the lack of correction plans for seven significant care and safety issues should raise concerns for families considering this facility.
