A licensure complaint, prompted by #CO35465 was completed on 4/2/24. Deficiences were cited. Based on record review and interview, the residence failed to be responsible for the coordination of resident care services with known external service providers, affecting one of four sample residents (#1). (Cross-Reference S810, S1146 and S2230)Findings include:1. ReferenceChapter VII regulations governing assisted living residences, part 2.21, defines "External services" as personal services and protective oversight services provided to a resident by family members or healthcare professionals who are not employees, contractors, or volunteers of the facility. External service providers include, but are not limited to: home health, hospice, private pay caregivers and family members.2. Resident #1 was admitted to the residence on 5/13/22 with a diagnosis of schizophrenia.Review of Resident #.. Based on record review and interview, the residence failed to develop a policy for unanticipated illness, injury and significant change of status from baseline, affecting 11 current residents. (Cross-reference S1146)Findings include: On 4/2/24 at 7:45 a.m., a policy for unanticipated illness, injury or change in baseline status was requested from the administrator designee.On 4/2/24 at 9:13 a.m., a document titled "person centered care plan" was provided. However, no policy for unanticipated illness, injury or change in baseline was ever provided.On 4/2/24 at 9:28 a.m., the house manager stated she provided the "person centered care plan" document, since she was unsure what a policy for a change in baseline was. The house manager stated whenever a resident experienced a change in baseline, they .. Based on record review and interview, the residence failed to ensure a comprehensive assessment was updated at least annually or whenever the residents' conditions changed from baseline status, affecting one of four sample residents (#1) who experienced a change in baseline status. (Cross-reference S810, S1160 and S2230)Findings include:1. ReferenceChapter VII regulations governing assisted living residences, part 12.7, requires that the comprehensive assessment shall include all the following items:(B) Information regarding the resident' s overall health and physical functioning ability;(E) Current diagnoses and any known or anticipated need or impact related to the diagnoses;2. Resident #1 was admitted to the residence on 5/13/22 with a diagnosis of schizophrenia.A pre-admissio.. Based on record review and interview, the residence failed to ensure resident records contained documented progress notes, by staff before the end of their shift, of out of ordinary events or issues that affected a resident' s physical, behavioral, cognitive, and/or functional condition, along with the action taken by staff to address that resident' s changing needs documented, affecting one of four sample residents (#1). A certification complaint, prompted by #CO35466 was completed on 4/2/24. A deficiency was cited. Based on record review, and interview, the facility (residence) failed to conduct an assessment whenever there was a significant change in physical, cognitive, or behavioral needs, affecting one of four sample residents (#1), who experienced a change in condition. Findings include:1. Resident #1 was admitted to the residence on 5/13/22 with a diagnosis of schizophrenia.A pre-admission assessment for Resident #1, dated 5/11/22, read in part that Resident #1 had no history of infections or physical health issues.A medical discharge summary, dated 1/21/24, read Resident #1 had a rash on the left calf, which showed a large area of raised scabs over individual papules. The rash did not appear infectious and could have been a form of vasculitis or bug bite reaction which was not usual for scabies. Permethrin 5% topical cream was prescribed.The residence' s most recent care plan, which was the same as the residence' s assessment, dated 3/8/24, read in part that Resident #1 had no skin issues. Review of Resident #1' s progress notes revealed no documentation of Resident #1 having had any skin changes or illness.2. InterviewsOn 4/2/24 at 9:28 a.m., the house manager (HM) stated Resident #1 went to a local urgent care facility toward the beginning of 2024 and returned to the residence without documentation of the visit. Further, the HM stated there were no written practitioner' s orders sent to the pharmacy although Resident #1 claimed he had a scabies skin infection and was prescribed medication to treat the infection. On 4/2/24 at approximately 9:30 a.m., the administrator designee stated the residence did not have a policy for unanticipated illness or change in baseline status.On 4/2/24 at 11:21 a.m., a representative at the local urgent care facility stated Resident #1 presented with an "itchy" rash in January 2024. The representative stated the urgent care practitioner who examined Resident #1 documented that the rash did not appear infectious and could have been bed bug bites or scabies.
