This facility is highly regarded for its physical environment and rehabilitation outcomes, but it has a history of serious, recurring complaints regarding staffing levels and the safety of personal property. When touring, we strongly recommend asking for a detailed plan on how they ensure patient safety in the pediatric unit and what specific protocols are in place to prevent the theft of resident belongings.

Key Review Excerpts

The facility felt like a hospital and they did not answer call bells, sometimes taking almost an hour. She developed a wound, fell out of bed and had special wipes brought in by the family used for other patients.

Rehab patient's family · 2019★★☆☆☆

My son was transferred to the pediatric unit about 2 months ago and the experience was quite stressful for us as he was only 3 months old at the time and needed a lot of sensitive care and adjustments for his growth. Pathways has been nothing but amazing when it has come to making this transition easier for us.

Pediatric patient's parent · 2024★★★★★

Pathways is a great facility. My family member has been here for over two months and the staff has treated us like family. The only recommendation I can make is that they are understaffed but the staff works very hard to make sure they live up to everyone’s expectations.

Pathways has faced recurring issues with safety systems, medication management, and care planning across multiple surveys, with families filing complaints about nursing staffing and abuse reporting. The facility shows a pattern of deficiencies in fire safety equipment, emergency preparedness, and resident assessment processes that have persisted from 2019 through 2025.
