What this means for your family

This facility has a highly regarded physical therapy department that has helped many patients recover mobility, making it a potential option for short-term rehab. However, the recurring reports of neglect, slow call-light response times, and hygiene issues are critical red flags. If you consider this facility, we strongly recommend unannounced visits and asking specifically about their current staffing ratios and how they handle patient hygiene and call-light response protocols.

Park Meadows Healthcare & Rehabilitation Center is a polarizing facility where experiences are sharply divided between those praising the physical therapy department and those reporting severe neglect. While many families and patients highlight the dedication of specific therapists and nursing staff, a significant number of reviewers report issues with facility cleanliness, slow response times to call lights, and poor communication. Families should be aware that while some find the environment 'family-oriented,' others have raised serious concerns regarding basic hygiene and patient safety.

Key Review Excerpts

The therapy team is amazing especially Mike and Patrick. I went to the hospital for 9 days and transfer to Park meadows Health and Rehab Center and I met those two guys. I could not walk, stand and did none on my own. It takes 1 1/2 week and I was able to stand, use the restroom by myself, and walk using a walker.

Rehab patient · 2024★★★★★

My loved one was left sitting in his own feces all night long after asking to be changed, only to be told, “We’ll be right back.” No one ever came.

Family member · 2025☆☆☆☆

The staff here have way exceeded my expectations and I couldn’t be happier with the care my mother has recieved. If I ever need a rehab / skilled nursing center I will definitely come here myself.
