What this means for your family

While the therapy department is frequently praised, the facility has a concerning pattern of hygiene lapses and medication errors. We strongly advise families to look elsewhere if the patient requires memory care, and if you proceed, ensure you have a plan for daily, in-person monitoring to address potential staffing and care gaps.

Optalis Health and Rehabilitation of Grand Rapids, formerly known as Samaritas, receives highly polarized feedback from families and residents. While some individuals praise the therapy department and specific staff members for their kindness, a significant number of reviewers report severe lapses in hygiene, medication management, and basic patient care. Families should be aware of recurring complaints regarding understaffing, particularly on weekends and night shifts, which often leads to neglect and poor communication.

Key Review Excerpts

This place gave my loved one pills before dialysis! It was made clear to them that no medicine should be given prior to this invasive procedure. My loved one kept telling them no pills but they insisted on giving them to her anyway!

Family member of patient · 2024☆☆☆☆

They do not work with patients that have issues with dementia. They will say they do when taking on new patients to get revenue but once the patient starts having issues... they will give you a 30 day eviction notice.

Family member of patient · 2024☆☆☆☆

My dad has been living at the lodge since December... But you do need more help on 3rd shift and on the weekends because the amount of patients the facility has. You also need a new call system with lights outside the door so the staff know when patients need help.
