This was a Case Management visit following an incident report. One deficiency was cited under CCR 87468.1(a)(1) regarding the personal rights of residents. This deficiency is classified as Type B, requiring the facility to conduct staff training and submit proof of completion.

COMPLAINT INVESTIGATION REPORT

This report details a complaint investigation regarding a facility's alleged failure to provide a refund to a responsible party. The investigation found the allegation to be 'Unsubstantiated' based on the evidence reviewed, including payment ledgers and admission agreements. No specific deficiencies were cited in the report.

The facility underwent an unannounced Annual Required inspection. While the facility has emergency and infection control plans, and general areas were found to be in good repair, a significant deficiency was noted regarding staff qualifications. Specifically, multiple staff members were found to lack current First Aid and CPR certification, which poses an immediate risk to residents.

This report details a complaint investigation conducted regarding the facility's food service. The allegations, which included claims of inadequate food supply and staff eating resident food, were investigated. Based on the findings, the allegations were deemed unsubstantiated, and no citations were issued.

The facility was inspected following a self-reported incident regarding a resident elopement. Two deficiencies were cited, both related to the failure to provide adequate supervision, which was deemed an immediate risk to resident safety. A civil penalty of $500.00 is being assessed due to the lapse in supervision.
