FACILITY EVALUATION REPORT

The facility underwent an unannounced Annual Required – 1 yr. inspection. The inspection noted several positive observations, including proper food storage, functional safety equipment, and adequate staffing records. No deficiencies were cited during this inspection.

The inspection was a Case Management visit following an incident report. One deficiency was cited regarding the lack of an updated physician's report for a resident with dementia. This deficiency is classified as Type B, indicating a potential, but not immediate, risk that requires correction.

The unannounced Annual Required inspection found the facility generally compliant in several areas, including food storage, medication management, and resident room readiness. However, one deficiency was cited regarding structural integrity, specifically a damaged wall in the facility.

The inspection was a Case Management visit conducted unannounced to follow up on amending a document from a previous visit and reviewing a death report. No deficiencies were cited during this visit.

The inspection revealed multiple critical deficiencies across several areas, including fire safety, food service, and facility maintenance. Specifically, multiple exits were obstructed, expired/moldy food was present, and significant structural disrepair was noted. Furthermore, unsecured dangerous items like sharps and cleaning supplies were accessible to residents, necessitating immediate corrective action.
