The home lacked organized resident records; staff could not access the new electronic system (Alice) to retrieve patient demographics, medication information, or emergency contacts during EMS responses.

Employee files for Employees 1, 2, 3, and 4 did not contain evidence that initial tuberculosis (TB) testing was completed within the required 10-day timeframe after hire.
Resident A's medication administration record (MAR) indicated self-administration, but the service plan did not include this instruction.
Resident B missed a dose of alprazolam on 8/3/24; staff failed to document a reason for the missed dose.
Resident C missed multiple doses of various medications (allopurinol, liothyronine, etc.) on several dates in August and September 2024; staff failed to document reasons for the missed doses.
Service plans for Residents A and B did not accurately specify their medication administration status.
Resident B's service plan indicated self-administration of medications, but the MAR showed staff were administering certain medications (controlled substances).
Staff incorrectly documented for the entire month of August that they administered Resident B's cipro when the resident actually self-administers it.
