What this means for your family

While the physical therapy department is frequently cited as a strength, the recurring reports of severe neglect, hygiene failures, and slow response times are deeply concerning. We strongly advise families to conduct unannounced visits and speak directly with current residents' families before choosing this facility for a loved one.

Questions for Your Tour

Key Review Excerpts

I went to be admitted for Cardiac Rehabilitation after having open heart surgery post surgery 5 days. I got there to what boiled down to be a Nightmare from Hell!!! Blinds falling down, old heating and air system struggling to operate, inoperable and unsafe tv, no patient Call button ( I was given a cowbell).

Rehab patient · 2022☆☆☆☆

Myom has been there for 10 days with no bath not even a pan of water for her to wash. Called nurse or for help took 1 hour for someone to come. Nurses of aids do not work! One nurse for the entire floor.

Long-term resident's family · 2025☆☆☆☆

My mother was in and out of RMC because this "care" center kept letting a wound get reinfected, and also failed to see she had a fractured hip, had her walking on it.

This facility faced serious concerns when families filed complaints leading to an October 2024 survey that found deficiencies in abuse protection, resident rights, and behavioral health care. The most recurring problems involve fire safety systems and building maintenance issues that have persisted across multiple surveys since 2018. While all deficiencies show correction dates, the pattern of ongoing safety violations and recent complaint-triggered abuse protection failures warrant careful consideration during your visit.
