FACILITY EVALUATION REPORT

The inspection was a Case Management review following a reported medication error for Resident 1. The facility was cited for a Type A deficiency related to the significant delay in administering medication. The facility acknowledged the error and took disciplinary action, and the deficiency was noted as cleared for policy training conducted during the visit.

This report details a complaint investigation regarding allegations of staff failing to prevent scabies outbreaks and not providing necessary medical attention to residents. Both allegations were reviewed by the Licensing Program Analyst and ultimately found to be unsubstantiated due to a lack of preponderance of evidence.

The inspection was a Case Management follow-up regarding a self-reported incident of abuse. The Licensing Program Analyst conducted interviews and reviewed records, but no citations were issued during this visit.

COMPLAINT INVESTIGATION REPORT

The facility was inspected for case management purposes and no deficiencies were cited during this visit. The Interim Executive Director was informed that required documents to change the Administrator will be provided by EOB Thursday, 9/11/2025.

The facility was inspected by the Licensing Program Analyst (LPA) on 09/02/2025. The LPA observed the facility and noted that the facility was clean, at a comfortable temperature, and had no obstructed exits or insufficient food supplies. No deficiencies were cited during this visit.
