Key Review Excerpts

The moment I walked in the OVERWHELMING smell of urine was ATROCIOUS and I literally had to go back out to my car and grab an old mask and spray perfume in it just to be able to go inside to spend time with my grandfather.

Memory care family member · 2025☆☆☆☆

My mother was recently here at Monterey Park Rehab. What an amazing place. The nurses were so kind. Would like to mention Brian and Jeremy. My mother is pretty stubborn and they were able to work with her and get her to do her therapy.

Long-term resident's family · 2025★★★★★

My mom went in because she could not walk after a month of rehabilitation the doctor says she is ready to go home. Still using a hoyler lift for transferring from wheelchair to bed. My mom turns the call light on and it takes for ever for someone to come and check on her.

7deficiencies
Near state avg (12.0)

This facility shows concerning patterns with families filing multiple complaints, particularly about medication management which has resulted in repeated deficiencies across surveys. The most frequent problem areas include fire safety systems, medication errors, and resident rights violations including failure to properly notify families of incidents. While the facility corrects most issues when cited, the recurring nature of medication problems and the fact that families have filed formal complaints about care quality suggest ongoing operational challenges that prospective families should carefully investigate.
