The following deficiencies were found during the on-site compliance inspection conducted on January 29, 2025:

If an assisted living facility provides medication administration, a manager shall ensure that:R9-10-816.B.3.b

Based on record review and interview, the manager failed to ensure a medication administered to a resident was administered in compliance with a medication order, for one of two residents sampled who received medication administration. Findings include: 1. A review of R1's medical record revealed a service plan, dated November 13, 2024, for directed care services including medication administration. 2. A review of R1's medical record revealed a list of medication orders, dated December 27, 2024, which included the following: - "Atenolol 50 mg tablet, Take 1 tablet (50 MG) by oral route once daily hold for SBP<160 or HR<60." 3. A review of R1's medical record revealed a Medication Administration Record (MAR) dated January 2025. The MAR documented the following: - "Atenolol 50 MG 1-PO BID," was marked, "Hold SBP <100 or HR <60." However, the order was to hold for a systolic blood pressure less than 160. 4. A review of R1's medical record revealed a chart documenting R1's blood pressure and heart rate twice daily during the month of January 2025. The chart documented R1's systolic blood pressure had never exceeded 160 during the month of January 2025, however, the medication had only been marked held for systolic blood pressures less than 100 or heart rate less than 60. 5. In an interview, E1 reported the previous order was to hold Atenolol for systolic blood pressure less than 100. During the on-site inspection, E1 reported R1's doctor had confirmed the current orders were incorrect and would need to be changed. 6. In an interview, E1 acknowledged R1's medical record did not document medication had been administered to R1 as ordered.

A manager shall ensure that food is obtained, prepared, served, and stored as follows:R9-10-817.C.4.a

Based on observation and interview, the manager failed to ensure foods requiring refrigeration were maintained at 41\'b0F or below. Findings include: 1. During an environmental tour of the facility, the Compliance Officer observed a refrigerator in a storage room adjacent to the kitchen. The refrigerator contained foods requiring refrigeration, such as applesauce, margarine, milk, cream cheese, and orange juice. However, a thermometer in the refrigerator read 48\'b0F. 2. In an interview, E1 acknowledged foods requiring refrigeration had not been maintained at 41\'b0F or below.

Based on this deficiency-free compliance inspection, the Department shall not conduct a compliance inspection for twenty-four months, according to A.R.S. \'a7 36-425(E). Subsection (E) does not prohibit the Department from enforcing licensing requirements as authorized by A.R.S. \'a7 36-424.
