What this means for your family

While the rehabilitation team is highly regarded, the recent reports of staff unprofessionalism and communication failures are concerning. We strongly recommend that families tour the specific room their loved one will occupy rather than relying on a model room, and ask for a direct line to the nursing station to ensure accessibility.

Mohawk Meadows receives high praise for its friendly, attentive staff and effective rehabilitation services, with many families reporting a warm and welcoming environment. However, recent reviews from 2025 and 2026 highlight serious concerns regarding communication, hygiene, and unprofessional behavior by nursing staff. Families should be aware of the discrepancy between the facility's 'model' rooms and the actual living conditions provided to residents.

  • 2What specific steps has the facility taken recently to address and improve medication management protocols?
  • 3Since some families have mentioned difficulty reaching the team by phone, what is the best way for us to stay in consistent communication with the nursing staff regarding our loved one?

  • 5How does the nursing team handle medical emergencies or sudden changes in health during the overnight hours?

Key Review Excerpts

Ryan from the social service department was the absolute biggest help he went above and beyond helping me with any concerns or questions n making a safe n secure discharge plan for my sister to come home

Long-term resident's family · 2024★★★★★

I found the place to be a little run down though. When we were looking at places we went to MM and they showed us a really nice room with a window and furniture, nicely painted etc. so we felt it would be nice but in reality, what we ended up with was FAR different.

Family member · 2025★★☆☆☆

My mom is at Mohawk Meadows. I don't understand how they can have no phones in the room and then no one answers the phone when you call the nurses. They actually hang up the phone on you.

Mohawk Meadows has faced multiple family complaints triggering investigations, with serious issues including resident protection failures and medication management problems that have persisted across surveys. The facility shows recurring deficiencies in fire safety systems, resident care planning, and pharmacy services, with some fire safety violations appearing repeatedly from 2021 through 2024. While all cited issues have correction dates, the pattern of recurring problems and complaint-driven investigations suggests ongoing quality concerns families should carefully evaluate.
